1. Who may submit

A family account must be created and managed by a player’s parent or legal guardian. Athletes do not receive separate accounts. The online film-evaluation service remains available only for athletes age 13 or older and must be submitted by a parent or legal guardian for players ages 13–17.

2. Information collected

The family portal collects parent contact information, general location, communication preferences, referral source, and emergency-contact information. Athlete profiles may include full name, preferred name, date of birth, grade, school, height, playing history, positions, apparel sizes, recurring schedule conflicts, goals, and volleyball interests.

The separate evaluation form collects information needed to provide a film review. The website does not request payment-card information or a home street address. Please do not enter medical diagnoses or other unnecessary sensitive details in open-response fields.

3. How information is used

Family and athlete information is used to manage profiles, plan practices and tournaments, register athletes for opportunities, communicate with parents, prepare for emergencies, understand family interests, and support future program services. Evaluation information is used to provide coaching feedback and document authorization. Information is not sold.

4. Evaluation permission and publishing permission

Permission to receive and privately review footage is required to provide the evaluation. Permission to reproduce, edit, or publish footage is separate and optional. Declining public use does not affect eligibility for the free evaluation. For a minor, optional publishing permission must be granted by the parent or legal guardian.

5. Footage rights

The submitter must confirm that they recorded or own the footage, or otherwise have the right to submit it and authorize the selected use. Permission concerning a player’s likeness does not necessarily grant copyright permission for video owned by a tournament, livestream provider, club, school, or another person.

6. Service providers and access

Parents access the portal through secure sign-in managed by the website platform. Server-side ownership checks restrict each parent to athletes connected to their own account. Family and athlete records are available to Coach Matt for program administration but are never displayed on public player pages. The website’s hosting and storage provider processes submitted records and files. Resend processes limited details when an email notification is needed.

7. Retention and deletion

Private submission records and uploaded footage are retained only while reasonably needed for the evaluation and related communication, with a routine maximum of 180 days from submission. They may be deleted earlier upon a verified request. If public use was separately authorized and content was created, a minimal authorization record may be kept while that content remains published or as reasonably needed to document the permission.

A parent, legal guardian, or adult player may request access, correction, withdrawal of optional media permission, or deletion by emailing mattbohorquez@gmail.com. Coach Matt may request reasonable information to verify the requester’s relationship to the submission. Deletion requests include the private database record and any uploaded original file under Coach Matt’s control.

8. Security

Reasonable administrative and technical measures are used to limit access and protect submitted information. No online service can guarantee absolute security, so submitters should avoid including unnecessary identifying information in footage or notes.

9. Changes and contact

This policy may be updated as the evaluation service changes. Material changes will be reflected on this page with a revised effective date. Questions or privacy requests may be sent to mattbohorquez@gmail.com.
